Job Description
Family Connections is an approach developed and tested over more than 25 years in the USA.
Family Connections is grounded in prevention and intervention sciences with a focus on supporting families to reduce risk factors and increase protective factors in the family environment.
Family Connections uses a strength based, systems orientated approach, offering in-home family support, that involves an individual families’ assessment and outcome driven service plan.
We are looking for a dynamic and experienced Team Leader to provide leadership and coordination support to the Family Connections team in Brisbane and Caboolture. This position will involve providing support and coaching to staff, coordination of referrals, decision making and problem solving with the Family Connections Team. This will position entails connecting and developing working relationships with key stakeholders, such as Child Safety & QLD Health. This role will ensure that all families engaged in the Family Connections Program are provided with outreach home visiting services, alongside the use of the Common Assessment Tool (CAT) to develop individualised strength based goals and support plans that consider the needs of all members of the family.
This is a full-time employment opportunity until June 2028 at Level 7 of the SCHADS Award.
Please note this position covers two regions - Teams are based in Brisbane (Hawthorne) and Caboolture (Women & Girls Health Hub) Weekly travel between sites is essential.

Key Responsibilities:

Essential Criteria
Tertiary qualification in Social Work or Human Services or related field
Minimum 3 years experience in a leadership role within a community service program, providing support and case coordination to families with complex needs.
Demonstrated experience in providing coaching and mentoring to case workers.
Demonstrated engagement and interpersonal skills particularly with those families who may be involved in the Child Protection system.
