Job Description

The Hospital Partnerships Team Leader oversees three integrated programs within the Hospital Discharge Team: Working Together to Connect Care (WTTCC) and the two Homefront teams based at Mater and Princess Alexandra Hospitals, ensuring coordinated support and consistent service delivery across all sites.

The Hospital Discharge Team works in partnership with major Brisbane hospitals including Royal Brisbane and Women’s Hospital, The Prince Charles Hospital, Mater Hospital, and Princess Alexandra Hospital to support people who frequently present to Emergency Departments or experience repeated hospitalisations. Through programs such as Working Together to Connect Care (WTTCC) and Homefront, the team provides integrated clinical and social support, with a strong focus on improving access to healthcare, strengthening community connections, and supporting long-term housing stability.


We’re looking for an experienced and values-driven Team Leader to guide the Hospital Discharge Team, supporting staff and working closely with hospital and community partners to deliver coordinated, high-quality care for people with complex needs.


This is a fixed term (parental leave cover) full time employment opportunity until June 2027 at Level 6 of the SCHADS Award.

Desired Skills and Experience

Key Responsibilities:

  • Lead and support a multidisciplinary team, fostering a positive, trauma-informed and collaborative environment
  • Assess, prioritise and allocate referrals across hospital partnerships and programs
  • Facilitate allocation meetings, team meetings, and contribute to care coordination with internal and external stakeholders
  • Support staff through mentoring, guidance, and day-to-day operational leadership
  • Provide direct support to participants with complex needs, including access to healthcare and housing
  • Maintain strong partnerships with hospitals and external organisations to ensure continuity of care
  • Oversee data accuracy, reporting, and compliance with organisational requirements
  • Contribute to workforce wellbeing, workload management, and a safe working environment
  • Support recruitment, onboarding, and performance development of staff
  • Ensure adherence to risk management, clinical governance, and safeguarding practices

Essential Criteria

  • Minimum 3 years’ experience supporting people facing vulnerability and marginalisation
  • Experience leading multidisciplinary teams
  • Ability to contribute to strategic direction and lead within complex service environments
  • Experience managing clinical incidents and applying risk management frameworks
  • Ability to build and maintain effective partnerships with internal and external stakeholders
  • Experience supporting staff development, training, and performance
  • Demonstrated commitment to safeguarding, cultural diversity, and safe work practices
