Job Description
Young Mothers for Young Women (YMYW) offers peer and professional support to young pregnant and parenting women, 20 years and under, their children and families who reside in the Moreton Bay region.
We are seeking a dedicated and experienced Senior Practitioner to join our Young Mothers for Young Women (YMYW) team in Caboolture. This role involves providing targeted family support through outreach strategies to young pregnant and parenting women, their children, and partners. The successful candidate will promote a culture of respect, safety, and trauma-informed approaches.
This is a permanent full time employment opportunity with conditions and salary at Level 5 of the SCHADS Award.
Desired Skills and Experience
Key Responsibilities:
Provide outreach and home visiting support, coordinated case management, and advocacy.
Conduct holistic assessments of families’ needs and develop support plans.
Triage referrals and allocate cases in collaboration with the Team Leader.
Empower families through participation and access to resources and services.
Apply the Parents as Teachers (PAT) framework and child development home visiting program.
Support young women in accessing legal, health, and community services.
Advocate for young women experiencing domestic and family violence.
Assist families in navigating systems such as child protection, family law, and housing.
Maintain high-quality case notes and support plans.
Requirements:
Relevant tertiary qualification in human services and at least 4 years’ experience.
Understanding of issues faced by young pregnant and parenting women.
Experience in assertive outreach and home visiting using a case work model.
Ability to provide information, referral services, and participate in group work.
Commitment to social justice and organisational policies and procedures.
Effective communication and advocacy skills.
Experience working with Aboriginal and Torres Strait Islander peoples and culturally diverse backgrounds.
IT competencies, including Microsoft Office.
Desirable:
Knowledge of children’s development and the service system for young families in Brisbane.
Current First Aid Certificate.
